Meet the Members of Ascend’s Hospice Care Team

At Ascend Hospice, our highly trained team of healthcare professionals and volunteers work together to ensure that each patient receives compassionate and individualized, hospice and palliative care. We are available 24 hours a day, 7 days a week, based on patient-centered, identified needs.

The hospice care team is comprised of the following individuals, each of which plays an important role in the care of the patient.

Family Member(s)

  • Acts as the primary caregiver of the patient
  • Works with the hospice care team to develop a plan for treatment
  • Coordinates a plan with the hospice care team to provide personal care and emotional support

Attending Physician

  • Certifies patient’s terminal prognosis
  • Directs pain and symptom management
  • Prescribes appropriate symptom management and comfort/palliative care
  • Co-manages the plan of care with hospice team

Medical Director

  • Leads hospice team in the development of the plan of care
  • Certifies terminal prognosis
  • Provides hospice care consultation services to physicians

Social Worker

  • Assists with end-of-life planning
  • Assesses patient and family members’ emotional and financial needs
  • Develops a personalized plan of care to meet patient and family members’ needs
  • Provides counseling to patient and family members
  • Arranges referrals to the appropriate community agencies for patient and family members

Registered Nurse

  • Specialized training in pain and symptom management
  • Assesses and attends to patient and family members’ needs
  • Coordinates the plan of care with hospice team
  • Ensures identified needs of patient and family are met
  • Provides support and education to patient and family members

Hospice Aide

  • Provides personal care and comfort measures to the patient
  • Provides emotional support to patient and family members
  • Reports identified needs to hospice-care nurse

Volunteers

  • Provide companionship, holistic therapies, and support to patient and family members
  • Provides non-medical services and respite to the caregiver(s)
  • Provides support at time of death and during bereavement

Chaplain

  • Assesses patient and family members’ spiritual needs
  • Develops a plan of care to meet identified needs of patient and family members
  • Provides counseling to patient and family members
  • Provides hospice care consultation to community clergy
  • Provides bereavement support

Bereavement Counselor

  • Administers pre-bereavement assessment
  • Provides individual and group counseling
  • Offers bereavement assistance up to 13 months following loved one’s death
